چکیده
abstract: herpes simplex encephalitis (hse) is a life threatening outcome of herpes simplex virus (hsv) infection of the central nervous system (cns). hsv accounts for 2-5 percent of all cases of encephalitis. one third of cases occur in those younger than 20 years old and one half in those older than 50 years old. clinical diagnosis is recommended in the encephalopathic, febrile patients with focal neurological signs. however, the clinical findings are not pathogonomic because numerous other diseases of cns can mimic hse. diagnosis should be confirmed based on medical history, analysis of cerebrospinal fluid (csf) for protein and glucose contents, the cellular analysis and identifying the pathogens by serology and polymerase chain reaction (pcr) amplification . the diagnostic gold standard is the detection of hsv dna in the cerebrospinal fluid by pcr. but negative results need to be interpreted regarding the patients clinical signs and symptoms and the time of csf sampling. spike and slow wave patterns is observed in electroencephalogram (eeg). neuroimaging, especially magnetic resonance imaging (mri) is essential for evaluating the patients, which shows temporal lobe edema or hemorrhage. all patients with hse should be treated by intravenous acyclovir (10mg/kg q8hr for 14-21 days). after completing therapy, pcr of the csf can confirm the elimination of replicating virus, assisting further management of the patient.

Fluent Aphasia From Herpes Simplex Encephalitis
The present case report introduces a patient with fluent aphasia, anterograde amnesia and anosmia due to herpes simplex encephalitis after her first delivery. The left medial temporal lobe was one of the main areas involved. On aphasia testing she showed severe anomia on both confrontation and free recall, agraphia, alexia, repetition disorder and some auditory comprehension impairments. Therap...

We presented four cases diagnosed with Herpes simplex virus encephalitis of those treated in our clinic in 2006 one of whom died, one recovered without any sequels. Two cases were left with advanced motor mental sequel. In this paper, clinical and laboratory findings along with diagnosis and treatment of Herpes simplex virus encephalitis were discussed.

A 28-year-old woman presented with a one day history of high fever and partial seizures with secondary generalization. This was preceded by a three week history of headache, ataxia, and fatigue. An initial computed tomogram head scan showed a low density mass lesion in the right frontal operculum without enhancement. On the next day, a repeat scan showed a new frontopolar, expansile, low densit...

Introduction Herpes simplex virus encephalitis is one of the more common sporadic viral encephalitides with an estimated incidence of one case per half million individuals per year (1, 2), but the true incidence may be higher. Untreated it is associated with a mortality rate exceeding 70070 and a high morbidity amongst the survivors with only about 9070 returning to normal neurological function...

Herpes simplex virus infection of the central nervous system is still a significant cause of morbidity and often mortality. Changes of central nervous system are results of primary infection or activation of latent HSV-1, HSV-2. Neurological deficits often follow encephalitis herpetica. The application of PCR is prompt and specific diagnosis of herpes simplex virus infections of the brain. Adva...

Herpes simplex encephalitis is a rapidly progressive disease in neonates and adults. Mortality is high, and there are severe neurologic sequelae in survivors. The reasons for centripetal transfer of virus to the brain are not fully understood. Prompt diagnosis followed by antiviral therapy with acyclovir significantly improves the prognosis. Controversy surrounds the need for brain biopsy befor...
